Who provides Diabetes Care at Brooke Street Medical Centre?
Your Diabetes Team can be made up of:
You, at the very least would have a GP & Diabetes Educator support you in managing your diabetes.
What can I expect with my diabetes care?
In order to provide you with the best possible care, you can expect Brooke Street Medical Centre Diabetes Team to do the following, as a minimum:
The team is here to assist you with your understanding of diabetes and managing such things as food choices, activity levels, blood glucose monitoring, etc.
Your GP and Diabetes Educator will conduct a thorough assessment to determine your personal needs and create a care plan with you accordingly.
A Team Care Arrangement may be suggested to ensure the thorough co-ordination of your care where a number of service providers are involved in your care.
What if I am a patient newly diagnosed with diabetes?
Our Diabetes Educator is available, with your GP, to spend time helping you understand this condition and what to do to care for yourself. We understand that this can be a very stressful time and aim to tailor treatment to meet your personal needs.
What is the cost of this service?
Most appointments will be linked with a GP and therefore eligible for a Medicare rebate. Please enquire at Reception for further fee information.Diabetes Australia, Victorian website:
